¹To submit this order for pest management services, your service agreement must be executed in person at time of inspection and/or initial service requested, and execution of agreement must take place prior to any services actually being performed. Order is valid for properties within service area. All services performed will be based upon executed service agreement referred to above. You also consent to have PayPal process your payment card information, which will be authorized for the amount of inspection fee. Inspection fee will be applied to initial service charges, provided that service is executed within 30 days of inspection. All fees are due and payable at time service is rendered, your credit card will be billed via PayPal for balance of fees onsite by Pest Rx. In the event that you choose to cancel your order; or are otherwise unable to comply with these terms, no services will be performed and your inspection fee will be refunded; provided that your request for cancellation occurs at least 24 hours prior to inspection date. If your cancellation notice is not received prior to inspection date, a $25 cancellation fee will be billed to your payment card. Pest Rx will endeavor to schedule your appointment on the date you specify. In the event requested date is full, the earliest date available will be offered.
